Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.Name of Product: Holiday-themed tea light candle warmersUnits: About 5,700Importer: Nantucket Distributing Co. Inc., of Middleboro, Mass.Retailer: Christmas Tree Shops, of Union, N.J.Hazard: The candle warmers can catch fire because the warming bowl is positioned too close to the tea light candle, posing fire and burn hazards.Incidents/Injuries: The firm has received one report of the Snowmen candle warmer catching on fire causing a minor burn injury to a consumer’s hand.Description: This recall involves holiday-themed tea light candle warmers in the shapes of snowmen and gingerbread houses. A warming bowl with wax fragrance tablets is suspended above the tea light candle. The Snowmen model has SKU/UPC 000016005396 and the Gingerbread house model has SKU/UPC 000016005372 printed on a label on the bottom of the product. - The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.Name of Product: 5 Hour Tea LightsUnits: About 10,000 casesDistributed by: The Sterno Group LLC, of Des Plaines, Ill.Hazard: The tea light wax can overheat resulting in the wax catching fire, posing a burn and fire hazard.Incidents/Injuries: Sterno has received several reports of the flame from the tea lights catching the wax on fire. No reports of injuries have been received.Description: The recall involves Sterno Bulk Pack 5 Hour Tea Lights, item numbers 505TL and 505S. The tea lights are standard sized, white wax tea light candles contained in small aluminum cups with no additional identification markings. The tea lights were sold in cases containing 500 tealight candles. Each case contains 10 boxes with 50 individual tea light candles. This recall only involves tea lights with codes ranging from 11060 to 11151 which can be found printed on both the outside of the case and on the back of the box of 50.Sold through: Food service distributors and other restaurant supply stores nationwide from April 2011 through October 2011 for between $36 to $50 per case. The recalled tea lights were not sold to retail consumers.Manufactured in: ThailandRemedy: Stop using these candles immediately and return any unused candles to the place of purchase for a full refund. Of the 65 incidents, 28 of them, including 37 burn injuries and two fatalities, occurred with fuel gel products made by Napa Home & Garden, which conducted a recall of its products in June 2011, in cooperation with CPSC. Also in June, CPSC issued a press statement alerting consumers to the hazards of pourable gel fuels. All pourable gel fuel, regardless of manufacturer, poses flash fire hazards.Consumers should not attempt to use or fix pourable gel fuel bottles with homemade remedies, or replace the fuel with other flammable materials.Retailers should stop sale of existing inventory and immediately remove all stock of pourable gel fuel from shelves. Eleven of these reported damage to surrounding surfaces and furnishings. Two minor burns and one puncture wound have been reported.Description: The product is QVC item H189749, a metallic spinning candle holder with 12 soy tea lights. The candle holders come in four colors - red, green, silver and gold. The candle holder has a base that holds a tea light and a shade which is attached to the center of the base by an approximately six inch vertical metal rod. Consumers should immediately take the toy’s jars and caps away from children and dispose of any jar lids without vent holes. Only use jars that have lids with vent holes.This children’s product was originally recalled in January 2009. Since that time, there have been additional injuries caused by the Spa Factory Spa Fantasy Aromatherapy Fountain & Bath Benefits Kits. Pressure from the buildup of carbon dioxide in the jars of Bath Bombs/Balls or Bath Fizzies that come with the kits can cause the unvented lids to blow off, posing explosion and projectile hazards. The flying pieces also can cause property damage. Additionally, the mixture of water with the Bath Bombs/Balls or Bath Fizzies can create citric acid. This acid can get into consumers’ eyes when the jars explode, posing a risk of eye irritation.As of January 2009, CPSC had received 88 reports of exploding jars, including 13 injuries to children. Since that time, CPSC has received 12 additional reports of exploding unvented jars of JAKKS’ Bath Bombs/Balls or Bath Fizzies, including 13 additional reported injuries. Hazard: The candle flame can unexpectedly flare up and the glass container can crack, posing fire, burn and laceration hazards to consumers.
Incidents/Injuries: The firm has received five reports of the glass breaking and six reports of high flames. No injuries or property damage have been reported.
Description: This recall involves DayNa Decker Botanika brand candles sold in 3-, 6-, and 16-ounce glass containers. The words "DayNa Decker" and Bacara, Bardou, Clementine, Indigo, Leila, Manzanita, Sierra, Taiga, Violetta, Viva, Yasmin, or Zelia appear on the bottom of the green glass container. Only Botanika candles with batch code 9J3, 9K1, 9L1 or 9L2 are included in the recall. Batch codes are printed on the bottom of the glass container.
Sold at: Resorts, spas, gift shops and specialty stores nationwide from November 2009 through January 2010 for between $30 and $75, depending on the size.

Problem: These votive glass candle holders can break unexpectedly during use.
Incidents/Injuries: Southern Living at HOME has received 15 reports of glass votive candle holders breaking unexpectedly. There are no reported injuries or damage.
Description: This recall involves the Old Taylor Estate Candelier with item number 40697 (the item number is located on one side and the top of each box). The chandeliers have eight glass votive candle holders. Old Taylor Estate Candeliers purchased after September 2005 are not included in this recall.
Sold at: Southern Living At Home consultants sold these chandeliers including the glass votive candle holders at home parties nationwide from June 2005 through September 2005 for about $80.

Incidents/Injuries: The firm has received one report of a cinnamon stick igniting and causing a small fire contained to the candle. No injuries have been reported.
Description: The recalled candle is 4-inches in diameter and 6-inches tall with a long wick that ties two cinnamon sticks to the top of the candle. The candle is brown and cream in color. The item number (CS406BR) and "CinnSpc Cndl 4x6 Brown $19.99," and "Made in Thailand" are printed on the label on the bottom of the candle.

Hazard: The wick can move from the center of the jar to the side causing the glass to overheat and possibly crack or shatter. Exposure to broken glass and molten wax poses laceration and burn hazards to consumers.
Incidents/Injuries: Old Williamsburgh Candle Corp. has received 11 reports of the candles cracking or shattering, including one laceration injury.
Description: This recall involves all scents and colors of Mason Jar Candles in 3.5, 4.0, 4.5, and 5.0 oz. sizes. Retailer: Sally Foster, Inc., of Troy, Mich.
Importer: Innovage Distribution, Inc., of Los Angeles, Calif.
Hazard: The tea light candles have a clear, plastic shell that can melt or ignite, posing a fire or burn hazard to consumers.
Incidents/Injuries: Sally Foster has received two reports of the plastic shells of these tea light candles igniting, causing minor property damage. No injuries have been reported.
Description: The recalled tea lights were sold as part of the three-piece Glass Candle Holders with Tea Lights Set, item number S106 in the Sally Foster catalog and Web site. The product's packaging was marked "Votive Holders with Tealights-Set of 3" and "Item Number 2006 157." Only the white tea light candles with clear plastic shells are affected. Products delivered after December 8, 2006 included tea lights with metal shells and are not included in this recall. The glass candle holders sold with these tea lights are not subject to the recall and consumers may continue to use them.
Sold through: Sally Foster's Web site from July 2006 through December 2006 and in Sally Foster catalogs from August 2006 through December 2006 for about $15.

Name of Product: Pumpkin-Shaped Candle Holders

Retailer: QVC Inc., of West Chester, Pa.
Distributor: Arteflorum Inc., of South San Francisco, Calif.
Hazard: The product can crack, break or shatter when used with the tea light candles that accompany them, yielding sharp pieces of glass that pose a laceration hazard.
Incidents/Injuries: QVC has received 85 reports that the product cracked, broke or shattered during use. Two consumers reported receiving minor lacerations that did not require medical treatment to their hands.
Description: The product is the "Heartfelt 3 Glass Pumpkins with Tealights and Garland by Valerie." It consists of three separate glass candle holders of three graduated sizes which are shaped like pumpkins. Each set includes six tea light candles, as well as a glass bead decorative garland. The pumpkin candle holders were sold in either clear glass or an amber color.

Sold by: QVC through television, its Web page, its toll-free number, and in its outlet, employee and Studio stores from September 2006 through November 2006 for between $45 and $50 plus about $8 for shipping and handling.
Remedy: QVC has already notified each customer by telephone and mail to stop using the candle holders until they received replacement tea lights. Name of product: MPL, multi-purpose utility lighter
Units: 101,557
Manufacturer: Zippo Manufacturing Company, of Bradford, Pa.
Hazard: The utility lighters may spill fuel from the nozzle when first used, causing a potential fire hazard through a secondary source of ignition.
Incidents/Injuries: There have been 27 reports that the butane fuel spills out of the nozzle when first used. There have been no reports that the product ignited or caused any injury. This recall is being conducted to prevent the possibility of injury.
Description: The Zippo MPL (Multi-Purpose Lighter) uses refillable butane fuel, is child resistant, and measures 8 inches long and 1 inch wide. The utility lighter is finished in black metal or silver metal with a chrome nozzle. The black unit is packaged in a blister pack with black and red graphics and the silver unit is in a silver gift box. Only utility lighters with these date codes are recalled: G03, H03, I03, ZH03. The date code is on the black adjuster knob on the bottom of the lighter.
Sold at: The Zippo Multi-Purpose Lighters were sold at Lowes, Bed Bath & Beyond, select Yankee Candle stores, Barbeque Galore, Harrington and Hammacher Schlemmer Catalog, select Wal-Mart stores and other convenience, hardware and home, hearth and patio stores from October 2003 to September 1, 2004.

Remedy: Free replacement multi-purpose lighter when consumer sends to the company the black adjuster knob from the bottom of the lighter.

Return Procedures: Consumers who own the Zippo MPL (Multi-Purpose Lighter) with date codes G03, H03, I03, ZH03 should immediately stop using this product and remove the black adjuster knob with the date code. Pull the knob straight downward with your fingers or use pliers to remove the knob. Keep the knob to be returned to Zippo Manufacturing Company. Consumers should dispose of the lighter in a manner that is in compliance with state or local requirements. Do not incinerate or puncture the lighter. Do not return this lighter to the store where purchased or to Zippo.

Hazard: 5-gallon plastic pails containing the cleaning products can unexpectedly crack and leak from the base, posing a risk to consumers due to the corrosive nature of these products.
Incidents/Injuries: Acuity Specialty Products Group Inc. has received three reports of leaking pails. No injuries or property damage have been reported.
Description: Zep Industrial Purple Cleaner & Degreaser is a clear, purple liquid. Zep Heavy Duty Floor Stripper is a clear, colorless, water-thin liquid. Both products were sold in 5-gallon, white pails with large blue labels and blue lids. Each pail carries a batch code written on the side of the pail. Consumers with a leaky pail should place it inside another 5-gallon pail or large plastic bag, and then pour the remainder of the contents into a new container and label the product. Use gloves and eye protection when handling. Clean any spilled material by absorbing onto a solid absorbent and sweep up and discard it into a solid waste container. Rinse the affected area with clean water until no residue remains.

The pots have a raised floral and garland design.

Incidents/Injuries: Pottery Barn Outlet has received one report of a flare up. There are no reported injuries. During the previous recall, there were 10 reports of candleholders flaring and one consumer reported receiving a minor burn from the candle wax.
Description: The recalled candleholders are constructed of black wire and stand about 18 inches high. The tree branches hold six tea light candles in orange glass pots.

Hazard: The candles' wax can catch fire, causing a high flame. This can cause nearby combustibles to catch fire and result in burns to consumers.

Description: The recalled candles consist of a glass votive cup surrounded by a silver holder with a standing angel figure attached. Each holder contains an engraved quotation. The angel stands about 3 inches tall. Each candle has a proverb engraved into the metal holder.

Hazard: The lighters fail to meet federal safety standards because they lack child-resistant mechanisms. If young children gain access to these multi-purpose lighters, they can pose a fire hazard or burn risk.
Incidents/Injuries: Aristo has not received any reports of incidents or injuries involving the Fire Stick lighters.
Description: These refillable, butane-fueled lighters are silver in color with a black sliding switch at the middle of the product. Two models of the lighters are included in this recall: a 7-inch model (model number 324007) and a 12-inch model (model number 324012). "Aristo Home & Garden" and "Fire Stick" are printed on the products' original packaging.
Sold Through: Home and garden retail outlets nationwide from November 2002 through October 2004. The 7-inch models were sold for $10, and the 12-inch models were sold for $20.

Problem: Some of these tea light candles from one production run can burn with a high flame, due to contamination.
Incidents/Injuries: Southern Living At HOME has received one report of high flame. There are no reported injuries.
Description: This alert is only for consumers who purchased Item Number 40491 (the item number is located on the side of the packaging below the logo), an eight pack of Fern Warm Comforts tea light candles, after October 26, 2004. The tea light candles are green and come in a plastic holder. Candles purchased prior to October 26, 2004 are not affected by this alert.

Hazard: The candle has a clamp that is used to extinguish the candle. The clamp does not always sever the wax close to the wick and extinguish the candle as intended. This can allow the flame to continue down into the coiled section of the candle and ignite the entire candle.
Incidents/Injuries: TechnoBrands Inc. has received two reports of the candle continuing to burn past the point where it is supposed to extinguish. TechnoBrands, Inc. has received no reports of property damage or injury.
Description: The rope candle is a 20-foot coiled beeswax candle that feeds up to a scissors like clamp. The candle is lit above the clamp. The candle has an antique copper finish. It was sold under the name "Self-extinguishing Rope Candle."

Hazard: The center of the candle has an unexpected high flame and the plastic petals could ignite resulting in melting and dripping plastic. The ignited plastic could also spread to nearby combustibles and pose a fire hazard to consumers. The battery contained in the middle of candle could eventually explode.
Incidents/Injuries: There have been no reported incidents or injuries.
Description: The candleholder is shaped like a lotus flower. It is plastic and consists of eight small candles that are ignited by a center flame. The small candles open up to resemble a flower blooming. There is a battery inside the center candle that allows it to play the music to "Happy Birthday".
Sold By: Skylighter sold the candles on their Web site only in sets of two for about $6.

Name of Product: Beeswax Rope Candle (SKU # 34-911)

Manufacturer: Zhongshan Zhongnam Candle Manufacturer Co., Ltd. of China
Hazard: The candle has a clamp (vise) that leads the end of the rope candle at the top. When clamped properly it is supposed to self-extinguish the candle when the burning wick meets the metal clamp (vise). The clamp (vise) does not always sever the wax close to the wick and does not always enable the wick to self-extinguish. This can lead to the flame continuing down into the coiled section of the candle, which can then ignite and cause fire damage.
Incidents/Injuries: Three incidents were reported where the candle continued to burn past the point where it is supposed to self-extinguish. There was some property damage in two incidents; however, nobody was hurt.
Description: The Rope Candle is a 21-foot coiled beeswax candle that feeds up to a scissors-like vise. The candle is lit above the vise. The candle is 7-inches tall and sits on an antiqued copper-finish holder.
Sold at: Gardener's Supply retail store in Burlington, Vt., and in the Holiday 2004 Gardener's Supply Catalog sold the candles from October 20 through November 12, 2004.

Remedy: The firm has designed a scissor like attachment that fits on top of the clamp (vise). This cuts the wax from the sides and extinguishes the flame, preventing the candle from burning below the attachment and igniting the entire candle. The firm will either replace the candle with the new fitting, give store credit for the original cost of the rope candle or give a full refund.

Description: The scented candle is housed in a clear glass holder and has a crumbled appearance with color variations. It contains slices of dried apple and cinnamon sticks in the wax. The candle is 3 inches in diameter and 3.5 inches high. "Kitchen Krumblers" is labeled at the base of the candle. The candle was sold separately or in a set with another candle not involved in this recall.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Walgreen Co., of Deerfield, Ill., is voluntarily recalling about 5,000 pumpkin tealight candleholders. The beads on the candleholders can ignite, posing a fire hazard.
Walgreens has received three reports of the beads on these tealight candleholders catching fire. No injuries have been reported. The "Colors of Autumn Beaded Pumpkin Tea Light Holders" are about 7 inches high and have amber-colored plastic beads covering a copper- colored metal frame. A black and copper-colored stem acts as a handle on the top of the product. The product was sold with a hang tag that contained the item number: 642146.
Walgreens stores nationwide sold the tealight candleholders from September 2003 to October 2003 for $9.99.

Hazard: These novelty lighters were imported without first being tested to show compliance with CPSC requirements that the lighters be child-resistant. They pose fire and burn hazards to young children.
Incidents/Injuries: None reported.
Description: There are two styles of lighters included in this recall. Nibo Space 5, model 12679, is a silver-colored lighter. The lighter is 2 7/8" tall and is in the shape of a spaceship. The word "SPACE" appears on one side of the lighter and the word "NIBO" appears on the other side. Nibo Space 19, model 13919, is a metal lighter that resembles a toy. The lighter is 3" tall. The word "NIBO" appears in a raised circle on one side of the lighter.
Sold through: The lighters were sold through the firm's catalog from April 2003 to September 2003 for about $20. The lighters were also given away as premiums in conjunction with purchase.
Manufactured in: China
Remedy: Consumers should stop using these lighters immediately. Consumers should dispose of the lighter in a manner that is in compliance with any state or local requirements. The lighters should not be incinerated or punctured. WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Sassi America Inc., of Elk Grove Village, Ill., is voluntarily recalling about 72,000 bottles of acrylic primer used to prepare fingernails before applying artificial nails. This methacrylic acid primer is not packaged in child-resistant packaging, as required by the Poison Prevention Packaging Act.
Sassi America has received a report of a 2-year-old girl in Pompano Beach, Fla., who sustained severe external and internal chemical burns after swallowing the liquid primer.
The primer is packaged in amber-colored bottles with black caps. The model number 508, 509, 510, 511 or 512 is written on the front of the label. "SASSI" also is written on the labels. The primer was sold in .25-ounce, .50-ounce, 1-ounce, 2-ounce and 4-ounce size bottles. The acrylic primers were sold separately and in kits.
Grocery and beauty supply stores nationwide sold the primer from June 2000 through January 2004 for between $2 and $10.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Home Interiors and Gifts Inc., of Carrollton, Texas, is voluntarily recalling about 211,000 sets of tea lights. Flames from the tea lights can flare, and the excessive heat can cause the plastic holders to melt, posing a fire hazard.
Home Interiors and Gifts has received 22 reports of the tea lights flaring up and melting their plastic holders. These incidents have resulted in minor property damage. No injuries have been reported.
The recalled tea lights are packaged 12 candles per box with "Home Interiors" written on the top of the box. Each tea light candle has a clear plastic base and is either red or ivory in color. The tea light candles are imported from Hong Kong.
Home Interiors' direct sales associates exclusively sold the recalled tea lights from September 2002 through November 2002 for about $5 per box.
Consumers should stop using the tea lights immediately and return any unused tea lights by mail to Home Interiors at 2901 Trade Center Drive, Suite 100, Carrollton, TX 75007. Consumers should include their return address information to receive a retail gift certificate from Home Interiors for the full purchase cost and shipping cost of the tea lights. The lighters were available in both 7-inch- and 14-inch-long sizes. The lighters were sold in a clear plastic sleeve and do not have any labeling on either the lighter or package.

PRODUCT: Christmas Candles - Blue Star Candles, L.P., of Lubbock, Texas, is voluntarily recalling about 18,000 Christmas candles. The 6-ounce, red Christmas candles are packaged in an 18-count box labeled "Christmas Candles." Each candle sits in a 6-ounce tin container. A label on the bottom of the candle reads in part, "Merry Christmas" with either a Santa Claus or Christmas tree printed on the candle. The candles have a cinnamon spice fragrance named Christmas Joy. Only the 6-ounce candle size is included in the recall.
PROBLEM: The 6-ounce candles can have the wrong size wick, causing the flame to burn irregularly, posing a fire or burn hazard.
INCIDENTS/INJURIES: Blue Star Candles, L.P. has received five reports of the candles burning irregularly. There have been no reports of injuries.
WHAT TO DO: Consumers should stop using the 6-ounce Christmas candles immediately and return any unused candles to Blue Star Candles, L.P. at 505 32nd Street, Lubbock, TX, 79404. WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Young's J.K. Inc., of Portland, Ore., is voluntarily recalling about 500 novelty cigarette lighters. These lighters do not have child-resistant mechanisms, as required by federal law. Young children can ignite the lighters, presenting fire and burn hazards.
CPSC has received one report of a 2-year-old using one of these lighters to ignite an upholstered chair. The fire resulted in the death of his 6-year-old brother and permanent brain damage to the 2-year old.
The refillable, dolphin-shaped cigarette lighters are butane gas- fueled and made of metal. There are three models involved in this recall. One of the lighters features a dolphin with a large ball and is labeled "1988.8 Young's Wholes 14.9." Another lighter features a single dolphin and is labeled "00005011 $14.00 EA." The last model features two dolphins and a ball, and has no label.
Gas stations, smoke shops, grocery, gift and liquor stores sold the lighters in Oregon and Southwest Washington from 1996 through October 2001 for between $8 and $20.

December 24, 2002, Release # 03-061; CPSC, Atico International Announce Recall of Christmas Candles

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Atico International USA Inc., of Fort Lauderdale, Fla., is voluntarily recalling about 60,000 Christmas tealight candles. The wick is not properly set inside the tealight, which may cause the plastic holder to melt and pose a fire hazard to consumers. Atico has received one report involving melting of the plastic holder; however, no injuries have been reported.
The recalled candles have a clear plastic holder and were available in red, white and green.

Eckerds, Kerr Drugs, Snyders Drugstore, and Farmacias El Amal stores nationwide sold the candles from September 2002 through December 2002 for between $2.50 and $4.
Consumers should stop using the candles immediately and return them to the store from which they were purchased for a full refund. WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Montrose Wholesale Candies & Sundries Inc., of Chicago, Ill., is voluntarily recalling about 247,000 disposable cigarette lighters. The lighters do not have child-resistant mechanisms, as required by federal law. Young children could ignite the lighters, presenting fire and burn hazards.
These "BIC" brand lighters were illegally imported into the United States by Montrose Wholesale Candies & Sundries, Inc., a Chicago importer. This recall does not involve lighters sold in the U.S. by BIC. In the United States, BIC sells only lighters with a child resistant mechanism. BIC notified CPSC of these illegally imported lighters, and CPSC investigators caught the non-child-resistant lighters for sale in several stores in the Chicago area. All disposable lighters imported and sold in the U.S. are required by law to be child-resistant.
CPSC and Montrose Wholesale Candies & Sundries are not aware of any injuries involving these lighters. This recall is being conducted to prevent the possibility of injuries.
The disposable cigarette lighters being recalled are oval-tube-shaped "BIC" and mini- "BIC" brand lighters. The lighters have a green, red, blue, black or yellow body and a metal top. "BIC" and "Made in France" are imprinted into the metal top of the lighter. A label on the lighter reads, "WARNING KEEP AWAY FROM CHILDREN." The recalled lighters are not equipped with a metal shield over the spark wheel. The child-resistant lighters have that shield. The recalled standard size lighters have the UPC number "3 086120 600020" and the mini-lighters have the UPC number "3 086120 600051."
Convenience, gas, and grocery stores in the central U.S. sold these disposable cigarette lighters from September 2002 through December 2002 for about $1.

November 20, 2002, Alert #03-510; CPSC, Meijer Announce Recall of Three Tier Tealight Candleholder

PRODUCT: Candleholders - Meijer of Grand Rapids, MI, is voluntarily recalling approximately 1,200 Three Tier Tealight Candle Holders. The holders are constructed of metal and glass disks and are in the shape of a vertical wave pattern. The tea light candles rest behind the glass disks. The holders are purple and black, yellow and white or green and white. Meijer stores located in Michigan, Ohio, Indiana, Kentucky and Illinois sold the candleholders from May 2002 through October 2002 for about $7.99. They were manufactured in China.
PROBLEM: The middle candleholder is mounted directly below the top candleholder, which can cause the top candle to overheat and flare. This poses a fire hazard, which can result in a burn injury or property damage.
INCIDENTS/INJURIES: There have been two incidents of candles flaring. The incidents did not result in injury or property damage. Meijer is recalling the candleholder to prevent injury or property damage.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Kohl's Department Stores Inc., of Menomonee Falls, Wis., is voluntarily recalling about 8,000 Halloween candleholders. Flames from the tea-light candle can ignite the candleholder, posing a fire hazard and risk of burn injuries to consumers.
Kohl's has received two reports of the candleholder igniting, causing smoke and soot damage to surrounding property. No injuries have been reported.
The recalled Halloween ghost candleholders are about 10- inches tall and are made of acrylic. The ghosts are white and hold a small orange pumpkin in which the tea-light candle sits. Two orange pumpkins are painted along the hem of the ghost. The candleholders were made in China.
Kohl's Department Stores sold the candleholders nationwide from August 2002 through October 2002 for about $17.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Pottery Barn, of San Francisco, Calif., is voluntarily recalling about 11,500 Halloween tree tea-light candleholders. There have been reports that some tea-light candles burning in the candleholder could flare up, posing a fire hazard and a risk of burn injuries to consumers.
There is an updated recall for this product in November, 2005.
Pottery Barn has received 10 reports of candles in the candleholder flaring. One consumer reported receiving a minor burn from the candle wax.The black wire "Scary Tree Tea-Light Holder" is about 18-inches high. The tree branches hold six tea-light candles in orange glass pots. The candleholder is made in China.
Pottery Barn stores, web site and catalogs sold the candleholders nationwide from August 2002 through early October 2002 for about $29.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Target Corp., of Minneapolis, Minn., is voluntarily recalling about 26,000 ceramic Jack-O-Lanterns. The flame of a candle placed inside the Jack-O-Lantern could ignite the straw ribbon on the outside, posing fire and burn hazards to consumers.
Target has received two reports of candles causing smoke and heat at the top of the Jack-O-Lantern, though no injuries or property damage has been reported.
The recalled ceramic Jack-O-Lanterns are shaped like a pumpkin and are five inches tall, six inches wide, and three inches deep. The tan colored pumpkins have an opening in the back and a straw ribbon attached to the stem of the Jack-O-Lantern. A label attached to the stem of the pumpkin reads, "OCTOBER HARVEST." The Jack-O-Lanterns were manufactured in China.
Target stores nationwide sold the Jack-O-Lanterns from September 2000 through October 2002 for about $5.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Pier 1 Imports, of Fort Worth, Texas, is voluntarily recalling about 9,200 candleholders. The Giant Champagne Glass-Shaped Floating Candleholder is made of mouth-blown recycled glass and is intended for use with floating candles. The candleholder can unexpectedly shatter and cause cuts to consumers.
Pier 1 has received two reports of the glass candleholder shattering. No injuries have been reported.
The recalled Giant Champagne Glass-Shaped Floating Candleholder is about 17 1/4-inches high by 13-inches in diameter. The green candleholders were made in Spain. The price sticker on the bottom of the candleholder reads "SKU#1884160."
Pier 1 Imports sold the candleholders through their retail stores and on-line nationwide from January 2002 through May 2002 for about $25.

PRODUCT: Lighters - S&G Enterprises of New York, N.Y. is voluntarily recalling about 9,700 Statue of Liberty cigarette lighters. The recalled lighters are dark green, metal, and are in the shape of the Statue of Liberty. The lighter's flame comes out of a hole in the top of the torch. The tablets in her left hand act as the ignition switch when pushed down alongside her body. The lighter distributed by S & G Enterprises, Inc. has the words "New York" embossed in gold. The patent number "6093016" is embossed in the same color as the body of the lighter at the back of the base. The packaging is a blue-gray paper box with a transparent plastic window in the front. "S & G Enterprises" and "Patent 6093016" are printed in black on either side of the packing box. A bar code with the numbers "6 63435 00221 8" and the price "$9.95" is printed on the bottom of the box. The lighters were sold nationwide from May 1999 to May 2001 for $9.95.
PROBLEM: The lighters do not have child-resistant mechanisms, as required by federal law. Young children could ignite the lighters, presenting fire and burn hazards.

These are refillable, gas-fueled, piezo-electric lighters in the shape of beverage cans. One lighter is labeled "Coca Cola" and the other lighter is labeled "Budweiser KING OF BEER." They are attached to keyrings.
Grocery, gift, and liquor stores, as well as smoke shops and gas station food marts in Oregon and southwest Washington sold these lighters from October 2001 through February 2002 for about $3.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), L.A. Lighter Inc., of Azusa, Calif., is voluntarily recalling about 1,800 disposable cigarette lighters. The lighters may have child-resistant mechanisms that do not meet federal safety standards. Young children may be able to operate these lighters, which could pose a fire hazard.
L.A. Lighter has not received any reports of incidents. This recall is being conducted to prevent the possibility of injuries.
The recalled lighters are made of metal, come in different colors, and have a top that resembles a pencil across a protractor and triangle. To open the port and ignite the flame, consumers press on the pencil. The lighters, which are about three inches high, 7/8 inches wide and 1/4 inch deep, have a ruler along the side. A label on the lighter has the UPC code 7 63052 20108 5 and the words, "MADE IN CHINA" and "SLIDE PUSH."
Retailers and distributors nationwide sold these lighters between December 2000 and May 2001 for about $5.

CPSC and Specialty Sales Co. Inc. have not received any reports of injuries or incidents. This recall is being conducted to prevent the possibility of injuries.
This recall involves two types of lighters. The first type is a refillable, liquid-fueled lighter with various decals depicting one of the following: a frog with a banjo sitting on a lily, a man in the moon, Tweety Bird, a unicorn, and a skull and crossbones. "Made in China" is written on a label on these lighters. The second type of lighter is a refillable, gas-fueled, piezo-electric lighter with a dolphin molded on the side. The dolphin's head serves as a hinged cover for the ignition button and flame port. There is no writing on these lighters.
Convenience and liquor stores and gas station food markets in northern California sold these lighters from March 1999 through April 2002 for about $4.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Island Soap & Candle Works of Honolulu, Hawaii, is voluntarily recalling about 29,000 candles. Pieces of the candles' wick can fall off while burning and the wick can re-ignite after extinguishing, posing a burn and fire hazard.
CPSC and Island Soap & Candle Works have received one report of a fire that caused minor property and smoke damage. No injuries have been reported.
The recalled candles have a single wick surrounded by wax in a brown coconut shell. They were sold in about 20 different scents and ranged in sizes from "Mini" to "Large." The coconut shell has a label on the bottom that reads "Island Soap & Candle Works...Hand made in Hawaii...Do not leave a burning candle unattended." Some of the candles have moon and star cutouts around the upper rim of the coconut shell.
Island Soap & Candle Works exclusively sold these candles in Oahu, Maui, and Kauai, Hawaii and Monterey, California from April 2000 through April 8, 2002 for between $8 and $15.
Consumers should stop using the candles immediately and return them to the store where purchased for a full refund. The recalled lighters are black with a chrome finished flame guard and a red gas control lever on the top. They are about six inches high and 1 3/8" in diameter and "TAIWAN" is imprinted on the chrome metal bottom. A half-inch metal screw facing back serves as an ignition button. The lighters were sold nationwide from December 2000 to May 2001.

Value City has received one report of the flowers in these candles catching on fire. No injuries or damage was reported.
The candles are made of uncolored wax and have dried yellow, purple and brown flowers in the outer layer. The candles are either square or round, and are about 4-inches high by 4-inches wide.

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Value City and Schottenstein stores, of Columbus, Ohio, are voluntarily recalling about 2,300 children's gel candle kits. When burned, the gel candle can melt the plastic candleholders included with the kit, posing a fire hazard to consumers.
CPSC has received two reports of consumers claiming the plastic candleholder melted and started a fire. One report involved substantial damage to the consumer's home, and a consumer reportedly suffered burns to his hands and feet. The other report involved minor damage to furniture.
The Gel Candles kit includes four bags of gel chips in bright colors, wicks, glitter, two glass holders, and two plastic candleholders. The candleholders are in the shapes of a bumblebee and a dragonfly, or a tulip and a daisy. The gel is scented in fruit flavors. The kits have model numbers "3041" or "3042" written on the box. Other writing on the box includes, "Express Ways!
